ABA Session Note Writing Guidance

  • Use observable and measurable language — describe what you see, not what you infer.
  • Include client's name, date, session duration, and location of services.
  • Document specific behaviors using operational definitions.
  • Record data collection method (frequency, duration, latency, etc.).
  • Describe the interventions implemented and client response.
  • Note progress toward treatment goals with specific percentages or trial data.
  • Avoid vague language like 'did well,' 'had a good session,' or 'was cooperative.'
  • Include any significant events, safety incidents, or protocol deviations.
  • Document caregiver involvement or training completed during the session.
  • Ensure notes are completed within 24 hours of service delivery.
