ABA Session Note Writing Guidance
- Use observable and measurable language — describe what you see, not what you infer.
- Include client's name, date, session duration, and location of services.
- Document specific behaviors using operational definitions.
- Record data collection method (frequency, duration, latency, etc.).
- Describe the interventions implemented and client response.
- Note progress toward treatment goals with specific percentages or trial data.
- Avoid vague language like 'did well,' 'had a good session,' or 'was cooperative.'
- Include any significant events, safety incidents, or protocol deviations.
- Document caregiver involvement or training completed during the session.
- Ensure notes are completed within 24 hours of service delivery.
